Fees & Insurance

I am not contracted with any insurance. Payment is due at the time of service. Prior to your appointment I will email you an invoice which you may pay online via credit (Visa / MC) or debit.

At the completion of your appointment, I will provide you with a superbill of services along with diagnosis and procedure codes.  This will allow you to submit a claim to your insurance for seeing a non-contracted provider.  

To provide as much transparency as possible prior to your appointment I have listed my fees below.  

APD testing fees:

Diagnostic APD testing: $1150

Estimating insurance reimbursement:

If you would like to get an estimate of the reimbursement you may receive from your insurance company, this is the question you should ask:

  • How much do you reimburse for the following codes if I see a non-contracted provider?  My provider is a Doctor of Audiology (AuD) and services were done in the 98115 zip code.

APD testing codes:

92620 - Evaluation of central auditory processing, with report; initial 60 minutes

Comprehensive diagnostic hearing test codes:

92557 - Comprehensive Audiometry Evaluation

92550 - Tympanometry and Acoustic Reflex testing

92587 - Otoacoustic Emission testing
